2008 Convening Expense Reimbursements
To facilitate participation in the Convening, predetermined expenses will be reimbursed for members of county, tribal, and state teams, as well as for facilitators, moderators, panelists, and presenters. The necessary forms to seek reimbursement are listed here.

All those seeking reimbursement must complete Form W–9, available here as an Excel form.

If you are a member of a county, tribal, or state team seeking reimbursement, complete this Excel form. If you are a member of a team, but also a facilitator, moderator, panelist, or presenter, you do not complete this form.

All facilitators, moderators, panelists, and presenters, including those who are part of a county, state, or tribal team, must complete this Excel form to submit reimbursement requests.
